Half of the country provides no foster care services for individuals aged 18 and older. As soon as they become legal adults, they age out of the system and find themselves forced to navigate, alone, a series of unfamiliar environments. Poor outcomes tend to arise as a result, including unemployment, homelessness, and incarceration.
Transitioning out of foster care can be challenging. Our specialized program supports young adults as they navigate this critical phase, providing housing assistance, educational support, job training, and life skills development to ensure a smooth transition to independent living.
We offer comprehensive GED preparation services to help individuals earn their high school equivalency diploma. Our program includes personalized tutoring, study resources, and practice tests to ensure that every student is well-prepared for the GED exam and future educational opportunities.
Our housing assistance programs, CYHOME and CYNERGY, provide tailored support for at-risk youth and young adults. We offer rapid rehousing, permanent supportive housing, and assistance with securing and maintaining stable housing.
CYNOVA, our employment support program, equips young adults with the skills and resources needed for successful career advancement. We offer job placement services, vocational training, resume building workshops, and career development guidance.
We partner with local second chance employers for training and placement.
CYNERVE, our educational support program, offers financial assistance for educational expenses, ESL classes, GED preparation, and skills development workshops. We aim to empower youth with the knowledge and skills needed for academic and personal success.
Through CYLINK, our day center and referral program, we provide a central hub for essential support services. Our outreach includes access to food, clothing, mental health services, education, housing, and employment resources.
We offer immediate assistance for urgent needs, including food, clothing, and hygiene supplies. Our emergency services ensure that clients have access to basic necessities during times of crisis.
Harm Reduction
Our harm reduction services focus on minimizing the negative consequences associated with high-risk behaviors. We provide safe use education, needle exchange programs, and overdose prevention to ensure the safety and health of our clients.
Emergency Service Plan (ESP)
We implement ESP to ensure that clients receive immediate and essential healthcare services during emergencies. Our team coordinates emergency care, stabilizes clients in crisis, and connects them with the necessary follow-up services.
Case Management
As a Medicaid provider, we offer extensive case management services to help clients navigate the healthcare system. This includes individualized care plans, referrals based on insurance, and comprehensive support to access medical and mental health services.

At Cornerstone Youth Collective (CYC), we serve as Address Confidentiality Program (ACP) Application Assistants, providing essential support to individuals affected by domestic violence. Our role includes assisting clients with the application process for the ACP, which helps protect their personal information and ensures their safety. We guide clients through the necessary paperwork, offer resources and referrals, and provide ongoing support to ensure their confidentiality and security. Our goal is to empower survivors by giving them the tools and assistance they need to maintain their privacy and rebuild their lives safely.
At Cornerstone Youth Collective, we are proud to serve as a designated provider for the Governor’s Opportunity for Supportive Housing (GOSH). This initiative is a crucial component of the State Hospital Discharge and Diversion (SHDD) Program, which is a collaborative effort between the Aging and Long-Term Support Administration (ALTSA) and Eastern and Western State Hospitals. Our role is to provide specialized housing search and tenancy support services to individuals seeking independent living.
